Indications

Omeprazole is a proton pump inhibitor (PPI) medication used to reduce the amount of acid produced in the stomach. It is effective in reducing symptoms such as sour stomach, belching, heartburn, and indigestion. This medication is indicated for the treatment of various conditions related to excess acid in the stomach, including gastric and duodenal ulcers, erosive esophagitis, gastroesophageal reflux disease (GERD), Zollinger-Ellison syndrome, and dyspepsia.

In some cases, omeprazole is combined with antibiotics to treat ulcers caused by the H. pylori bacteria. Omeprazole is also used to prevent upper gastrointestinal tract bleeding in critically ill patients. This medication is available in both over-the-counter (OTC) and prescription form from a healthcare provider.

Uses and Dosage

Omeprazole is available as a prescription medication in the form of a delayed-release capsule and granules for suspension, which can be taken orally or administered via a feeding tube. The over-the-counter form of omeprazole comes in the form of a delayed-release tablet.

Prescription omeprazole is typically taken once daily before a meal, although it may be taken twice a day when used with other medications to eliminate H. pylori, or up to three times a day for conditions involving excessive stomach acid production. Over-the-counter omeprazole is taken once a day in the morning for 14 consecutive days and may be repeated every 4 months if necessary. Take omeprazole at the same time each day to ensure consistent use.

The delayed-release tablets should be swallowed whole with a glass of water, while the delayed-release capsules should be swallowed whole or mixed with soft applesauce and consumed immediately. The granules for oral suspension must be mixed with water prior to use and consumed within 30 minutes. For those with a feeding tube, the granules for oral suspension may be given through the tube as directed by a doctor.

Follow the instructions on the prescription label or package carefully and to take omeprazole exactly as directed. Over-the-counter omeprazole should not be used for immediate relief of heartburn symptoms and should not be taken for longer than 14 days or more frequently than every 4 months without consulting a doctor.

Prescription omeprazole should not be discontinued without talking to a doctor, and if the condition does not improve or worsens, a doctor should be consulted. Over-the-counter omeprazole should not be used for more than 14 days or treated more frequently than once every 4 months without talking to a doctor, and if symptoms worsen or do not improve after 14 days, a doctor should be consulted.

Side Effects

Some side effects may occur that usually do not need medical attention:

  • body aches or pain
  • diarrhea or loose stools
  • difficulty with breathing
  • dizziness
  • ear congestion
  • gas
  • heartburn
  • loss of voice
  • muscle pain
  • runny or stuffy nose
  • sneezing
  • unusual drowsiness

Inform your healthcare provider immediately if you experience any serious side effects:

  • kidney injury, damage or failure
  • bone fracture of the hip, wrist or spine
  • vitamin B-12 deficiency
  • clostridium difficile-associated diarrhea (caused by intestinal infection)
  • lupus erythematosus
  • low magnesium levels

Caution

Prior to taking omeprazole, inform your doctor or pharmacist if you have any allergies, including a sensitivity to omeprazole itself or similar drugs such as esomeprazole, lansoprazole, or pantoprazole. Inform your healthcare provider of your medical history, particularly if you have liver disease or lupus.

If you experience any of the following symptoms, it is important to seek medical help immediately: lightheadedness, sweating, or dizziness accompanied by heartburn, chest pain, jaw pain, arm pain, or shoulder pain (especially if these symptoms are accompanied by shortness of breath or unusual sweating), or unexplained weight loss.

Use of proton pump inhibitors such as omeprazole may increase the risk of bone fractures, particularly with long-term use, higher doses, and in older adults. Your doctor or pharmacist can discuss strategies to prevent bone loss and fractures, such as taking calcium and vitamin D supplements.

Older adults may be more susceptible to the side effects of this medication, including bone loss and fractures, as well as C. difficile infection. Children may also be more susceptible to side effects, including fever, cough, and infections of the nose, throat, and airways.

During pregnancy, omeprazole should only be used if the benefits clearly outweigh the risks. It is important to discuss these risks and benefits with your doctor. Omeprazole is excreted in breast milk and its effects on nursing infants are unknown. Consult your doctor prior to breastfeeding.
