Indications

Sertraline is classified as a selective serotonin reuptake inhibitor (SSRI) and is used to treat a variety of mental health conditions. These include depression, obsessive-compulsive disorder, panic attacks, posttraumatic stress disorder, and social anxiety disorder.

This medication is also used to alleviate the symptoms of premenstrual dysphoric disorder such as mood swings, irritability, bloating, and breast tenderness. Sertraline helps to increase the levels of serotonin in the brain, a natural substance that plays a vital role in maintaining mental balance. By doing so, it helps to improve the symptoms associated with these conditions.

Uses and Dosage

Sertraline is available in tablet and liquid concentrate form for oral consumption. It is typically taken once daily, either in the morning or evening. For treating premenstrual dysphoric disorder, sertraline can be taken daily throughout the month or on certain days of the month.

Take sertraline at the same time every day, following the instructions on the prescription label carefully. Do not exceed or reduce the prescribed dosage or take it more frequently than directed.

Before use, the sertraline concentrate must be diluted. To do this, use the provided dropper to remove the prescribed amount of concentrate and mix it with 4 ounces of water or any of the specified liquids such as ginger ale, lemon or lime soda, lemonade, or orange juice. The diluted solution may appear cloudy, which is normal. Do not mix the concentrate with any other liquid and consume the diluted solution immediately.

Your doctor may initially prescribe a low dose of sertraline and gradually increase it, but not more than once a week. The full benefits of sertraline may take several weeks or more to manifest, continue taking it as prescribed, even if you feel better.

Do not discontinue sertraline without consulting your doctor. Abruptly stopping the medication may lead to withdrawal symptoms such as nausea, sweating, depression, mood changes, restlessness, irritability, confusion, dizziness, headache, fatigue, seizures, ringing in the ears, numbness or tingling in the extremities, difficulty sleeping, or staying asleep.

Side Effects

Some side effects may occur that usually do not need medical attention:

  • acid or sour stomach
  • belching
  • decreased appetite or weight loss
  • diarrhea or loose stools
  • heartburn
  • sleepiness or unusual drowsiness
  • stomach cramps, gas, or pain
  • trouble sleeping
  • agitation, anxiety, or nervousness
  • bladder pain
  • burning, crawling, itching, numbness, prickling, “pins and needles”, or tingling feelings
  • changes in vision
  • cloudy urine
  • constipation
  • difficult, burning, or painful urination
  • flushing or redness of the skin, with feeling of warmth or heat
  • frequent urge to urinate
  • increased appetite
  • pain or tenderness around the eyes and cheekbones
  • stuffy or runny nose

Inform your healthcare provider immediately if you experience any serious side effects:

  • decreased interest in sexual intercourse
  • failure to discharge semen
  • inability to have or keep an erection
  • loss in sexual ability, desire, drive, or performance
  • aggressive reaction
  • breast tenderness or enlargement
  • confusion
  • convulsions
  • diarrhea
  • drowsiness
  • dryness of the mouth
  • fast talking and excited feelings or actions that are out of control
  • fever
  • inability to sit still
  • increase in body movements
  • increased sweating
  • increased thirst
  • lack of energy
  • loss of bladder control
  • mood or behavior changes
  • muscle spasm or jerking of all extremities
  • nosebleeds
  • overactive reflexes
  • racing heartbeat
  • red or purple spots on the skin
  • restlessness
  • shivering
  • skin rash, hives, or itching
  • sudden loss of consciousness
  • unusual or sudden body or facial movements or postures

Caution

Prior to taking sertraline, inform your healthcare provider or pharmacist of any allergies you may have, including allergies to inactive ingredients present in the product, such as latex or tartrazine.

Before using sertraline, share your medical history with your healthcare provider or pharmacist, especially if you or a family member has a history of bipolar/manic-depressive disorder, bleeding problems, liver disease, seizure disorder, thyroid disease, or glaucoma.

Sertraline may cause a condition that affects the heart rhythm, known as QT prolongation, which can result in serious, and in rare cases, fatal, irregular heartbeat and other symptoms. This risk may be heightened in individuals with low levels of potassium or magnesium in their blood, as well as in those taking certain medications or experiencing conditions such as severe sweating, diarrhea, or vomiting.

Using sertraline may cause dizziness or drowsiness. Avoid engaging in activities that require alertness, such as operating machinery or driving, until you are confident that you can do so safely. Abstain from using alcoholic beverages or marijuana, as these substances can increase dizziness or drowsiness.

The liquid form of sertraline contains alcohol, and should be used with caution by individuals with diabetes, alcohol dependence, or liver disease. Certain medications, such as metronidazole and disulfiram, can cause a severe reaction when taken in combination with alcohol.

Older adults may be more sensitive to side effects, such as bleeding, loss of coordination, or QT prolongation. These individuals are also at a higher risk of developing hyponatremia, a type of salt imbalance, particularly if taking diuretics.

Children may experience side effects such as loss of appetite and weight loss. For children taking this medication, monitor weight and height.

Do not discontinue the use of this medication unless directed by your healthcare provider, as untreated mental or mood problems, such as depression, panic attacks, obsessive-compulsive disorder, and post-traumatic stress disorder, can be serious conditions.

During pregnancy, this medication should be used only when clearly needed. It may harm an unborn baby. Babies born to mothers who have used this drug during the last 3 months of pregnancy may rarely develop withdrawal symptoms such as feeding/breathing difficulties, seizures, muscle stiffness, or constant crying. If you notice any of these symptoms in your newborn, tell the doctor promptly.

Sertraline is known to pass into breast milk, consult with your healthcare provider before breastfeeding.
