Indications

Azithromycin is a macrolide antibiotic utilized in the treatment of specific bacterial infections such as bronchitis, pneumonia, sexually transmitted diseases (STDs), and infections affecting the ears, lungs, sinuses, skin, throat, and reproductive organs. It is also effective in managing or preventing disseminated Mycobacterium avium complex (MAC) infection, which is a type of lung infection that commonly occurs in individuals with human immunodeficiency virus (HIV). The mechanism of action of azithromycin involves halting the growth of bacteria.

Uses and Dosage

Azithromycin is available in tablet, extended-release suspension (long-acting liquid), and suspension (liquid) forms for oral administration.

Zithromax tablets and suspension are typically taken once daily with or without food for a period of 1-5 days. For the prevention of disseminated MAC infection, azithromycin tablets are generally taken once weekly, with or without food. The extended-release suspension is taken as a one-time dose on an empty stomach (at least 1 hour before or 2 hours after a meal). To ensure consistency, azithromycin should be taken at the same time each day. Adhere to the prescribed dosage and do not take more or less of it or take it more frequently than prescribed by your doctor.

Before use, shake the liquid well to ensure uniformity of the medication. Employ a dosing spoon, oral syringe, or measuring cup to measure the exact amount of medication. Rinse the measuring device with water after taking the full dose of medication.

If you receive a single-dose, 1-gram packet of azithromycin powder for suspension, you must first mix it with water before administration. Mix the contents of the 1-gram packet with 1/4 cup (60 mL) of water in a glass and consume the entire contents immediately. Add an additional 1/4 cup (60 mL) of water to the same glass, mix, and consume the entire contents to ensure that you receive the entire dose.

If you receive azithromycin extended-release suspension as a dry powder, you must first add water to the bottle before use. Open the bottle by pressing down on the cap and twisting. Measure 1/4 cup (60 mL) of water and add to the bottle. Close the bottle tightly, and shake well to mix. Use the azithromycin extended-release suspension within 12 hours of receiving it from the pharmacy or after adding water to the powder.

If vomiting occurs within an hour of taking azithromycin, contact your doctor immediately. Your doctor will advise you on whether or not to take another dose. Do not take another dose unless instructed to do so by your doctor.

It is expected that you will begin to feel better during the initial few days of azithromycin treatment. If your symptoms do not improve or worsen, consult your doctor.

Take azithromycin until the prescription is finished, even if you feel better. Do not discontinue the medication unless severe side effects are experienced. If you stop taking azithromycin prematurely or miss doses, your infection may not be fully treated, and bacteria may become resistant to antibiotics.

Side Effects

Some side effects may occur that usually do not need medical attention:

  • vomiting
  • stomach pain
  • headache

Inform your healthcare provider immediately if you experience any serious side effects:

  • fast, pounding, or irregular heartbeat
  • dizziness
  • fainting
  • rash with or without a fever
  • blisters or peeling
  • fever and pus-filled, blister-like sores, redness, and swelling of the skin
  • hives
  • itching
  • wheezing or difficulty breathing or swallowing
  • swelling of the face, throat, tongue, lips, eyes, hands, feet, ankles, or lower legs
  • hoarseness
  • vomiting or irritability while feeding (in infants less than 6 weeks old)
  • severe diarrhea (watery or bloody stools) that may occur with or without fever and stomach cramps (may occur up to 2 months or more after your treatment)
  • yellowing of the skin or eyes
  • extreme tiredness
  • unusual bleeding or bruising
  • lack of energy
  • loss of appetite
  • pain in the upper right part of the stomach
  • flu-like symptoms
  • dark-colored urine
  • unusual muscle weakness or difficulty with muscle control
  • pink and swollen eyes

Caution

Prior to taking azithromycin, inform your physician or pharmacist if you have an allergy to this medication, or to other antibiotics such as erythromycin, clarithromycin, and telithromycin, or if you have any other allergies. This medication may contain inactive ingredients that can provoke allergic reactions or other complications.

Before using this medication, provide your physician or pharmacist with your medical history, especially if you have a history of liver disease, kidney disease, or myasthenia gravis, a particular muscle disease.

Azithromycin has the potential to cause QT prolongation, which is a condition that affects the rhythm of the heart. In rare cases, QT prolongation can lead to serious (sometimes fatal) fast or irregular heartbeat, along with other symptoms such as severe dizziness and fainting, that require immediate medical attention.

Low levels of potassium or magnesium in the blood may also increase the risk of QT prolongation. This risk may be higher if you are taking certain drugs like diuretics, commonly referred to as “water pills,” or if you have conditions such as severe sweating, diarrhea, or vomiting.

Azithromycin may interfere with the effectiveness of live bacterial vaccines, such as typhoid vaccine. Inform your healthcare provider that you are taking azithromycin before receiving any immunizations or vaccinations.

Older adults may be more susceptible to the side effects of this medication, particularly QT prolongation.

During pregnancy, this medication should be used only when necessary, and the risks and benefits should be discussed with your physician.

This medication passes into breast milk. Consult your doctor before breastfeeding.
