Indications

Salmeterol serves multiple purposes in managing respiratory conditions. It is employed to regulate symptoms such as wheezing, shortness of breath, coughing, and chest tightness in individuals diagnosed with chronic obstructive pulmonary disease (COPD). It is also administered alongside an inhaled steroid medication to oversee these same symptoms in individuals with asthma. Salmeterol falls within the category of drugs known as long-acting beta agonists (LABAs). Its mechanism of action involves relaxing and widening the air passages within the lungs, thereby facilitating easier breathing.

Uses and Dosage

Always ensure the device is in a level, horizontal position before activation and use.

Follow your doctor’s instructions for inhaling this medication. Typically, you should use it twice daily, once in the morning and once in the evening, with a 12-hour gap in between. You may or may not taste or feel the medication when you inhale, and both experiences are normal. Never exhale into the device, and do not use it with a spacer. Avoid washing the mouthpiece or any component of the device.

If you are using other inhalers concurrently, allow at least 1 minute between the use of each medication.

The prescribed dosage is contingent on your medical condition and how you respond to the treatment. To maximize its benefits, use this medication consistently at the same times daily. Do not exceed the recommended dosage of one inhalation twice daily, as doing so may heighten the risk of side effects.

For occasional use in preventing exercise-induced asthma (bronchospasm), administer the medication at least 30 minutes before exercising. Wait at least 12 hours before using another dose. If you experience sudden asthma symptoms or shortness of breath, utilize a quick-relief inhaler like albuterol. Your doctor can provide more detailed instructions.

Familiarize yourself with which inhalers you should use daily (controller drugs) and which are for sudden worsening of symptoms (quick-relief drugs). Consult your doctor in advance to understand the appropriate steps to take if you develop a new or aggravated cough, increased shortness of breath, wheezing, greater sputum production, nighttime breathing difficulties, find yourself using your quick-relief inhaler more frequently (over 2 days a week), or if your quick-relief inhaler appears less effective. Learn when you can manage sudden breathing problems on your own and when immediate medical assistance is necessary.

Side Effects

Some side effects may occur that usually do not need medical attention:

  • joint pain

  • difficulty falling asleep or staying asleep
  • burning or tingling of the hands or feet

Inform your healthcare provider immediately if you experience any serious side effects:

  • fast or pounding heartbeat
  • chest pain
  • rash
  • hives
  • swelling of the face, throat, tongue, lips, or eyes
  • hoarseness
  • choking or difficulty swallowing
  • loud, high-pitched breathing

Caution

Prior to using this medication, inform your doctor or pharmacist about any allergies you have, as it may contain inactive ingredients that can provoke allergic reactions or other issues.

Share your medical history with your doctor or pharmacist, particularly if you have a history of heart problems (like angina or irregular heartbeat), high blood pressure, diabetes, liver problems, seizures, or thyroid problems (including an overactive thyroid).

Salmeterol has the potential to cause a heart rhythm disorder known as QT prolongation, which, though rare, can lead to serious fast or irregular heartbeat and other symptoms such as severe dizziness or fainting. Low levels of potassium or magnesium in your blood can further increase the risk of QT prolongation. This risk may be exacerbated by certain medications (like diuretics or “water pills”) or by conditions such as severe sweating, diarrhea, or vomiting. Discuss the safe use of salmeterol with your doctor in light of these considerations.

Older adults may be more susceptible to the side effects of this medication, especially with regard to QT prolongation (referenced above).

If you are pregnant, use this medication only when it is clearly necessary and after discussing the potential risks and benefits with your doctor.

It is currently unknown whether this medication passes into breast milk, so consult your doctor before breastfeeding if you are taking it.
