Indications

Pramipexole is indicated for the treatment of Parkinson’s disease either as monotherapy or in combination with other medications. It can effectively enhance motor function and reduce tremors, rigidity, bradykinesia, and postural instability. It may also alleviate the “on-off syndrome” in which patients experience intermittent periods of inability to move.

Pramipexole is additionally used to manage Restless Legs Syndrome (RLS), a medical condition characterized by an uncomfortable urge to move the legs, typically at night, resulting in disturbed sleep. Pramipexole is a dopamine agonist that helps restore the balance of dopamine in the brain, thereby mitigating these symptoms.

Uses and Dosage

Pramipexole is available in tablet and extended-release tablet forms for oral administration. In the treatment of Parkinson’s disease, the regular tablet is typically taken thrice daily, whereas the extended-release tablet is taken once a day. For restless legs syndrome, the regular tablet is taken once a day, 2-3 hours before bedtime. The extended-release tablets of pramipexole are not used for treating restless legs syndrome.

Pramipexole can be taken with or without food, and taking it with food may reduce the likelihood of medication-induced nausea. Take the medication exactly as directed by your doctor, do not take more or less of it or take it more often than prescribed.

Swallow the extended-release tablets whole; do not split, chew, or crush them.

Your doctor will initiate pramipexole at a low dose and gradually increase it as required, with increments typically not occurring more than once every 4-7 days. It may take several weeks to determine the appropriate dose.

If you are taking pramipexole for restless legs syndrome, be aware that your symptoms may worsen, occur earlier, or even occur in the early morning as your treatment progresses. Contact your doctor if your symptoms worsen or occur at different times.

Pramipexole alleviates the symptoms of Parkinson’s disease and restless legs syndrome, but it does not cure these conditions. Continue taking pramipexole even if you feel better. Do not stop taking pramipexole without first consulting your doctor.

Sudden cessation of pramipexole for Parkinson’s disease may cause symptoms such as fever, confusion, muscle stiffness, lack of interest or concern for usual activities, anxiety, depression, fatigue, sleep disturbances, sweating, or pain. Discontinuing pramipexole for restless legs syndrome may result in a worsening of symptoms. If you stop taking pramipexole for any reason, do not resume the medication without consulting your doctor, who will probably recommend gradually increasing your dose again.

Side Effects

Some side effects may occur that usually do not need medical attention:

  • nausea
  • weakness
  • dizziness
  • loss of balance, falling
  • difficulty falling asleep or staying asleep
  • difficulty remembering
  • abnormal dreams
  • heartburn
  • constipation
  • diarrhea
  • dry mouth
  • swollen, stiff, or painful joints
  • pain in back, arms or legs
  • frequent urination or urgent need to urinate
  • difficulty urinating or pain when urinating

Inform your healthcare provider immediately if you experience any serious side effects:

  • hallucinations
  • changes in vision
  • abnormal body movements and motions that you cannot control
  • changes in the way you sit or stand that you cannot control, such as your neck bending forward, bending forward at the waist, or tilting sideways when you sit, stand or walk,
  • dark, red or cola-colored urine
  • muscle tenderness
  • muscle stiffness or aching
  • muscle weakness

Caution

Prior to taking pramipexole, inform your physician or pharmacist of any allergies you have or any other allergies you may have. This medication may contain inactive components that can cause allergic reactions or other issues.

Before using this medication, discuss your medical history, particularly if you have low blood pressure, heart issues, kidney problems, mental/mood disorders, or sleep disorders (such as sleep apnea or narcolepsy), with your doctor or pharmacist.

This medication may cause dizziness or drowsiness. Alcohol or marijuana can amplify these effects. Do not operate machinery, drive, or perform any activities requiring alertness until you can do so safely.

Senior citizens may be more susceptible to dizziness and hallucinations while using this medication.

During pregnancy, only use this medication if it is necessary. Discuss the risks and benefits with your doctor.

This medication may enter breast milk and potentially have adverse effects on an infant who is nursing. It may also affect milk production. Do not breast-feed while using this medication without consulting with your doctor.
