Uses and Dosage
Nortriptyline is available in capsule and oral liquid forms for oral administration. The usual dosage regimen involves taking it one to four times a day, with or without food. Adhere to a consistent schedule by taking nortriptyline at approximately the same times each day. Take nortriptyline precisely as directed, without exceeding or reducing the dosage frequency prescribed by your doctor.
Typically, your doctor will initiate treatment with a low dosage of nortriptyline and gradually increase it over time.
Even if you experience an improvement in your symptoms, continue taking nortriptyline. Ceasing the medication abruptly without consulting your doctor may result in withdrawal symptoms such as headaches, nausea, and weakness. Your doctor will likely recommend a gradual reduction in dosage.
Nortriptyline is occasionally utilized in the treatment of panic disorders, post-herpetic neuralgia, and as an aid for smoking cessation.

Side Effects

Caution

Before using this medication, disclose your medical history to your doctor or pharmacist, particularly regarding any respiratory difficulties, liver problems, recent heart attack, urination difficulties, hyperthyroidism (overactive thyroid), personal or family history of glaucoma (angle-closure type), personal or family history of mental/mood disorders, family history of suicide, seizures, and conditions that may increase the risk of seizures (e.g., other brain diseases, alcohol withdrawal).
Nortriptyline has the potential to cause a heart rhythm disorder known as QT prolongation. In rare instances, QT prolongation can lead to serious rapid/irregular heartbeat and other symptoms (such as severe dizziness, fainting) that require immediate medical attention.
The risk of QT prolongation may be elevated in the presence of low levels of potassium or magnesium in the blood. This risk may also increase when using certain medications or experiencing conditions like excessive sweating, diarrhea, or vomiting. Discuss the safe use of nortriptyline with your doctor, particularly if any of these factors apply to you.
This medication may induce dizziness, drowsiness, or blurred vision. Consumption of alcohol or marijuana can intensify these effects. Refrain from driving, operating machinery, or engaging in activities that require alertness or clear vision until you can do so safely.
This medication has the potential to increase sensitivity to sunlight. Limit sun exposure, avoid tanning booths and sunlamps, and take precautions such as using sunscreen and wearing protective clothing when outdoors. If you experience sunburn or develop skin blisters or redness, promptly inform your doctor.
The liquid formulation of this medication may contain alcohol. Caution should be exercised if you have conditions such as diabetes, alcohol dependence, liver disease, or any other condition that necessitates alcohol restriction or avoidance in your diet.
For individuals with diabetes, this medication may complicate the management of blood sugar levels. Regularly monitor your blood sugar levels and inform your doctor of the results. Adjustments to your diabetes medication, exercise regimen, or diet may be necessary.
Elderly individuals may be more susceptible to the side effects of this medication, particularly dry mouth, dizziness, confusion, difficulty urinating, and QT prolongation.
During pregnancy, this medication should only be used if clearly necessary. Untreated mental and mood disorders such as depression, anxiety, or panic disorder can pose significant risks. Discontinuing the use of this medication should only be done under the guidance of your doctor. If you are planning a pregnancy, become pregnant, or suspect you may be pregnant, discuss the benefits and risks of using this medication during pregnancy with your doctor.
This medication passes into breast milk and may have adverse effects on a nursing infant. Prior to breastfeeding, consult your doctor for guidance and advice.
