Indications

Zoledronic acid is prescribed to address specific bone ailments, such as Paget’s disease, which causes bones to become abnormal and fragile. This drug is also utilized to treat osteoporosis in men and postmenopausal women who have a high risk of experiencing bone fractures. It can also be employed to prevent or treat osteoporosis in individuals taking corticosteroid drugs for an extended period.

Zoledronic acid works by slowing down bone degradation, thus promoting bone strength and minimizing the risk of fractures. This medication is categorized as a bisphosphonate.

Uses and Dosage

Zoledronic acid is administered as a liquid solution through injection into a vein, lasting at least 15 minutes. Typically, a healthcare professional administers this injection in a clinic, hospital, or doctor’s office.

If zoledronic acid is prescribed to treat cancer-related high blood calcium, it is usually administered as a single dose, with a second dose given at least a week after the first dose if the blood calcium levels remain elevated. When treating bone damage from multiple myeloma or cancer that has spread to the bones, zoledronic acid is usually given every 3 to 4 weeks.

If treating osteoporosis in postmenopausal women, men, or individuals taking glucocorticoids, a single dose of zoledronic acid is usually given once a year. To prevent osteoporosis in postmenopausal women, zoledronic acid is typically administered once every 2 years.

For Paget’s disease of bone, zoledronic acid is usually given as a single dose, with additional doses administered at a later time if needed.

Drink at least two glasses of water or another liquid before receiving zoledronic acid. During treatment, your doctor may recommend a calcium supplement and a multivitamin with vitamin D, which should be taken daily as directed. Inform your doctor if you cannot take these supplements for any reason during your treatment.

After receiving zoledronic acid injection, some patients may experience a reaction within the first few days. Symptoms may include flu-like symptoms, fever, headache, chills, and bone, joint, or muscle pain. These symptoms may last from 3 to 14 days, and your doctor may recommend taking a non-prescription pain reliever or fever reducer to manage them.

Even if you feel well, continue receiving zoledronic acid as scheduled if it is prescribed for the prevention or treatment of osteoporosis. Regularly speak with your doctor to determine if you still require this medication.

Side Effects

Some side effects may occur that usually do not need medical attention:

  • itching, redness, pain, or swelling in the place where you received your injection
  • red, swollen, itchy, or teary eyes or swelling around the eyes
  • constipation
  • nausea
  • vomiting
  • diarrhea
  • stomach pain
  • loss of appetite
  • weight loss
  • heartburn
  • mouth sores
  • excessive worry
  • agitation
  • depression
  • difficulty falling asleep or staying asleep
  • fever, chills, cough, and other signs of infection
  • white patches in the mouth
  • swelling, redness, irritation, burning, or itching of the vagina
  • white vaginal discharge
  • numbness or tingling around the mouth or in fingers or toes
  • hair loss

Inform your healthcare provider immediately if you experience any serious side effects:

  • rash
  • hives
  • itching
  • swelling of the eyes, face, lips, tongue, throat, hands, arms, feet, ankles, or lower legs
  • hoarseness
  • difficulty breathing or swallowing
  • upper chest pain
  • irregular heart beat
  • muscle spasms, twitches, or cramps
  • unusual bruising or bleeding
  • painful or swollen gums
  • loosening of the teeth
  • numbness or heavy feeling in the jaw
  • sore in the mouth or the jaw that does not heal

Caution

Before taking zoledronic acid, inform your doctor or pharmacist if you are allergic to it, to other bisphosphonates like alendronate or risedronate, or if you have any other allergies. This product may contain inactive ingredients that can cause allergic reactions or other issues.

Tell your doctor or pharmacist your medical history, particularly if you have kidney problems, low calcium levels in the blood, difficulty taking calcium or vitamin D supplements, recently had or planned dental surgery, gut problems such as malabsorption or surgery on the small intestine, received zoledronic acid treatment for cancer, or parathyroid or thyroid problems.

Some people may experience serious jawbone problems when using zoledronic acid. Your doctor should check your mouth before you start taking the medication, and inform your dentist before any dental work. Regular dental exams are necessary to prevent jawbone problems, and you must maintain healthy teeth and gums. If you experience jaw pain, inform your doctor and dentist immediately.

Before any surgery, particularly dental procedures, inform your doctor and dentist about this medication and all other products you are using, including prescription drugs, nonprescription drugs, and herbal products. Your doctor or dentist may advise you to stop taking zoledronic acid before surgery.

This medication may cause severe kidney problems, particularly in older people. Consult your doctor or pharmacist for more information. To avoid kidney damage, drink plenty of fluids unless instructed otherwise by your doctor.

Zoledronic acid can cause dizziness. Using alcohol or marijuana can exacerbate dizziness. Avoid driving, operating machinery, or performing activities that require alertness until you can do so safely.

Inform your doctor if you are pregnant or plan to become pregnant. Do not become pregnant while taking zoledronic acid, as it can harm an unborn baby. Discuss reliable forms of birth control while taking this medication and after stopping treatment.

Zoledronic acid can stay in your body for many years, so if you become pregnant, immediately speak to your doctor about the benefits and risks of this medication.

It is unknown if this medication passes into breast milk. Avoid breastfeeding while taking zoledronic acid and after stopping treatment, as it can harm nursing infants.
