Indications

Sildenafil is a medication used to treat erectile dysfunction (impotence), which is the inability to achieve or maintain an erection in men. This medication is also used to improve exercise capacity in adults with pulmonary arterial hypertension (PAH), a condition characterized by high blood pressure in the vessels that transport blood to the lungs, resulting in symptoms such as dizziness, shortness of breath, and fatigue.

Sildenafil belongs to a class of drugs known as phosphodiesterase (PDE) inhibitors. In the case of erectile dysfunction, sildenafil functions by increasing blood flow to the penis during sexual stimulation. For PAH, sildenafil works by relaxing the blood vessels in the lungs, which helps to improve blood flow.

Uses and Dosage

Sildenafil is available in tablet and suspension forms that are taken orally.

For erectile dysfunction treatment with sildenafil, adhere to your physician’s guidelines and the instructions in this section. Take sildenafil as needed before sexual activity. The ideal time to take sildenafil is approximately one hour before sexual activity, but it can be taken anytime from 4 hours to 30 minutes before.

Sildenafil generally should not be taken more than once every 24 hours. If you have certain medical conditions or are taking specific medications, your doctor may advise you to take sildenafil less frequently. You may take sildenafil with or without food.

If you are taking sildenafil to treat PAH, follow your doctor’s guidelines. You will likely take sildenafil three times daily, with or without food, at approximately the same times every day, spaced 4 to 6 hours apart.

Take sildenafil exactly as prescribed. Do not take more or less than prescribed, and do not take it more frequently than directed by your doctor.

Before each use, shake the liquid well for 10 seconds to mix the medication evenly. Use the oral syringe provided with your medication to measure and take your dose.

If you are using sildenafil to treat erectile dysfunction, your doctor will likely start you on an average dose of sildenafil and adjust your dosage based on your response to the medication. If sildenafil is not effective or you are experiencing side effects, inform your doctor.

If you are taking sildenafil for PAH, note that sildenafil manages PAH but does not cure it. Even if you feel well, continue to take sildenafil. Do not stop taking sildenafil without consulting your doctor.

Side Effects

Some side effects may occur that usually do not need medical attention:

  • diarrhea
  • flushing (feeling of warmth)
  • nosebleeds
  • difficulty falling asleep or staying asleep
  • numbness, burning, or tingling in the arms, hands, feet, or legs
  • muscle aches

Inform your healthcare provider immediately if you experience any serious side effects:

  • sudden severe loss of vision
  • blurred vision
  • sudden decrease or loss of hearing
  • ringing in ears
  • dizziness or lightheadedness
  • fainting
  • chest pain
  • worsening shortness of breath
  • erection that is painful or lasts longer than 4 hours
  • itching or burning during urination
  • rash

Caution

Prior to taking sildenafil, inform your doctor or pharmacist if you have any allergies or if this medication has caused an allergic reaction in the past. This product may contain inactive components that can lead to an allergic reaction or other health issues.

Before using this medication, let your doctor or pharmacist know about your medical history, particularly if you have experienced heart problems such as a heart attack or life-threatening irregular heartbeat in the past six months, chest pain or angina, heart failure, stroke within the past six months, liver disease, kidney disease, high or low blood pressure, penis conditions such as angulation, fibrosis/scarring, Peyronie’s disease, a history of painful or prolonged erection (priapism), or conditions that may increase the risk of priapism such as sickle cell anemia, leukemia, or multiple myeloma, eye problems such as retinitis pigmentosa, sudden decreased vision, or NAION.

This medication may cause dizziness or vision problems. Consuming alcohol or marijuana can increase the likelihood of experiencing dizziness. Do not operate machinery, drive, or perform any activity that requires clear vision or alertness until you can safely do so.

Sildenafil should only be used during pregnancy if it is essential. As high blood pressure in the lungs is a serious condition that can be detrimental to both the pregnant woman and the unborn child, do not stop taking this medication unless instructed to do so by your doctor. If you are considering pregnancy, are pregnant, or believe you may be pregnant, discuss the advantages and disadvantages of using sildenafil with your doctor.
