Indications
Lactulose is a synthetic sugar employed in the treatment of constipation. Once in the colon, it undergoes breakdown into substances that draw water from the body into the colon. This action results in the softening of stools, aiding in the relief of constipation. Lactulose can also be utilized to decrease the levels of ammonia in the blood of individuals with liver disease. It functions by attracting ammonia from the blood into the colon, facilitating its elimination from the body.
Uses and Dosage
If you are taking lactulose by mouth for liver disease, follow your doctor’s instructions, typically taking it 3 to 4 times a day. To enhance the taste, you can mix it into fruit juice, water, milk, or a soft dessert. The goal is to achieve 2 to 3 soft stools each day. The dosage is determined based on your medical condition and how you respond to the treatment, considering factors like the frequency of soft stools.
For constipation, take lactulose by mouth as directed by your doctor, usually once daily.
Lactulose can also be administered rectally as an enema for liver disease. Mix the recommended amount of lactulose with 700 milliliters (24 ounces) of water or normal saline. Administer the solution into the rectum, retaining the liquid inside for 30 to 60 minutes as directed by your doctor. If you retain the enema for less than 30 minutes, repeat the dose unless instructed otherwise.
Use this medication regularly to derive the maximum benefit from it. Take it at the same time each day.
When given rectally for liver disease, an improvement in mental status may be observed in as little as 2 hours. However, if you are taking lactulose by mouth, it may take up to 24 to 48 hours to notice an effect. If the medication is being used for constipation, it may take up to 48 hours to have a bowel movement. Inform your doctor if your condition persists or worsens.
Side Effects
Some side effects may occur that usually do not need medical attention:
- diarrhea
- gas
- nausea
Inform your healthcare provider immediately if you experience any serious side effects:
- stomach pain or cramps
- vomiting
Caution
- Before using lactulose, inform your doctor or pharmacist if you are allergic to it or if you have any other allergies. Inactive ingredients in the product may cause allergic reactions or other complications.
- Share your medical history with your doctor or pharmacist, especially if you have a low galactose diet, certain other bowel problems (bowel obstruction), or diabetes.
- This medication contains various sugars, and if you have diabetes, it may impact your blood sugar levels. Monitor your blood sugar regularly as directed and share the results with your doctor. Your doctor may need to adjust your diabetes medication, exercise program, or diet.
- During pregnancy, use this medication only when clearly necessary. Discuss the risks and benefits with your doctor.
- It is uncertain if this drug passes into breast milk. Consult your doctor before breastfeeding.

Cephulac doesn’t start to work until it reaches your gut. It can take 1 to 2 days to work and make you have a bowel movement.
